As a first step, we encourage you to speak directly with your health care provider or program facilitator to allow them the opportunity to work with you to resolve your concerns. Alternatively, you can fill out the form below to share your concern with us. If required, our Client Experience Coordinator will work with you to facilitate your concerns and assist in a resolution.

Client Rights and Privacy
In order to address a client or visitor concern, we will need information about the client or visitor and the care, service, or program that is of concern. Client privacy will be protected at all times. Chigamik has a legal responsibility to ensure that all personal health information collected, used and disclosed by Chigamik or its staff shall be treated in a manner that accords with the Personal Health Information Protection Act (PHIPA). Ideally, we will work directly with the client or with someone (usually a family member) who has the client’s consent to receive personal, confidential information about them.
