Under Ontario Health Teams, health care providers (including hospitals, community health centres, doctors and home and community care providers) work as one coordinated team.<\/p>\n<p>There are many Ontario Health Teams that have been established across Ontario, including the North Simcoe Ontario Health Team (NSOHT). The NSOHT works to create a connected health and social care system focused on what is most important to our local community.<\/p>\n<p>The NSOHT is comprised of seven anchor partners, inclusive of <a
