COVID-19 Self-Assessment

If you think you have COVID-19 symptoms or have been in close contact with someone who has it, please use this self-assessment to help determine if you need to seek further care: https://covid-19.ontario.ca/self-assessment. If you have an appointment at Chigamik and you have developed symptoms of COVID-19, please call reception at 705-527-4154 prior to your appointment and you will be directed to the COVID-19 assessment centre for testing.

COVID-19 Vaccinations Clinics

Information regarding who is currently eligible to receive a COVID-19 immunization and to self-schedule a vaccination appointment at a Community COVID-19 Vaccination Clinic is available on the Simcoe Muskoka District Health Unit website located here: www.simcoemuskokahealth.org/Topics/COVID-19/Vaccine-and-Immunization
 
A limited supply of AstraZeneca vaccines are available through the North Simcoe Family Health Team for individuals ages 40+. For more information and to book an appointment, please call 705-526-7804 ext 0 or visit www.nsfht.ca.
