The Indigenous COVID-19 Response Navigator’s role is to provide Indigenous community members, living in North Simcoe Muskoka, with supports or resources they may need to navigate the social and health care services during the COVID-19 pandemic, all within a culturally-safe environment.
Once referred to the Indigenous COVID-19 Response Navigator, an individualized care plan will be developed for the client and family members (if necessary). Follow up and ongoing support will also be provided.
Examples of reason to book an appointment with the Indigenous COVID-19 Response Navigator include, but are NOT limited to the following:
- COVID-19 concerns (mental health, employment, financial, screening/testing, education)
- Social determinants of health barriers
- Liaison and advocacy for any social and health care services
- Income/financial issues
- Legal/justice issues
- Employment/working conditions
- Education/home schooling
- Childhood Development
- Access to health care services
- Social supports
- Food security
How to Book an Appointment
Referrals can be made by your primary health care provider, or a self-referral can be made by contacting the Indigenous COVID-19 Response Navigator:
*Referrals should include the client’s name, contact information and a brief description of the reason for referral. The Indigenous COVID-19 Response Navigator will be in contact with the client within 1-3 business days.
