Thank you for your interest in becoming a client of Centre de santé communautaire Chigamik Community Health Centre.
Chigamik provides bilingual and culturally-sensitive primary care and community programs and services to meet the needs of our priority population which include Indigenous (First Nations, Métis, Inuit) and Francophone members of the community, and individuals who may be facing barriers to care. Chigamik services are available to those residing in our primary and expanded catchment areas throughout the North Simcoe Muskoka LHIN (see map below).
At this time, Chigamik is NOT accepting new clients at our main clinic in Midland. Chigamik will remain open for new client intake at our satellite clinic that runs out of the Georgian Nurse Practitioner-Led Clinic located at Georgian College. This clinic is open one day per week and is available for those who self-identify as Francophone.
If you are Francophone and interested in becoming a client at our Barrie satellite clinic, or if you would like to be placed on our wait list, please fill out the Client Intake Form and return it to Chigamik reception in Midland.
Our intake status is reviewed monthly and changes from time to time. Please check back periodically for updates.
As always, our programming continues to be available to all members of the community at no cost, regardless of your primary care affiliation. We encourage you to sign up for our community programs through our online registration system. Click here to view our current programs being offered.
