Ontario Health Team

Ontario Health Teams are being introduced to provide a new way of organizing and delivering care that is more connected to patients in their local communities. Under Ontario Health Teams, health care providers (including hospitals, community health centres, doctors and home and community care providers) work as one coordinated team.

There are many Ontario Health Teams that have been established across Ontario, including the North Simcoe Ontario Health Team (NSOHT). The NSOHT works to create a connected health and social care system focused on what is most important to our local community.

The NSOHT has a long-standing history of collaboration and is comprised of four core organizations, inclusive of Chigamik Community Health Centre, Georgian Bay General Hospital, North Simcoe Family Health Team, and Wendat Community Programs.

The NSOHT partners collaborate with patients, families, caregivers and the community to co-design the best possible care for our region.
