CHIGAMIK Community Health Centre is proud to be part of Ontario’s Locally-Driven Population Health Model, a provincial initiative designed to improve access to care for people who may face barriers to health and social services.
The model focuses on trusted community outreach, service navigation, partnerships, and local solutions that help connect people to the right care and supports, closer to home.
Through this funding, CHIGAMIK has expanded its capacity to support individuals and families in North Simcoe by adding two Community Ambassadors and a Nurse Practitioner to the team.
The Nurse Practitioner supports this work through care coordination, health system navigation, and collaboration with CHIGAMIK and community partners. This role is not open to the public for direct appointments.
The Community Ambassadors work directly with community members to help reduce barriers, build connections, and support access to local programs, services, and resources.
Community Ambassadors help connect people to the services, programs, and supports they need to improve their health and wellbeing.
At CHIGAMIK, Community Ambassadors work one-on-one with individuals to understand their needs, interests, and goals. They provide one-on-one support and can assist with a wide range of needs, including:
Connecting to primary care, including support with Health Care Connect
Accessing basic needs such as food, housing, and financial supports
Completing forms and applications
Navigating services and advocating for your needs
Connecting to social and wellness programs in the community
This work is based on social prescribing, a person-centred approach that connects people to local supports based on what matters most to them.
Aidan is one of CHIGAMIK’s Community Ambassadors, helping individuals and families connect to care, services, and community supports. Aidan works with clients to better understand their needs and help them find the right resources.
April is one of CHIGAMIK’s Community Ambassadors, supporting community members with service navigation, connection, and advocacy. April helps individuals access programs and supports that can improve their health, wellbeing, and sense of belonging.
