Community Outreach

Chigamik’s Community Health Worker connects with individuals in North Simcoe who are living on the street or at-risk of becoming homeless, to develop ongoing relationships, provide system navigation, individual advocacy, resources and referrals. Our Community Health Worker is able to help with a range of issues, including housing, food insecurity, income supports, community service referrals, access to harm reduction supplies (clean needles and safe inhalations kits) and one-on-one support.
