Our primary healthcare team takes the time to listen to your concerns and works together with you to come up with the best care possible. This may include connecting you with other healthcare providers on our team, enrolling you in one of our free community programs, or connecting you to other resources in the community.
Chigamik offers many healthcare services and programs under one roof. Our multi-disciplinary allied healthcare team made up of mental health counsellors, system navigators, physiotherapist, dietitian, health promoter, doulas, traditional healers, among others, are available to assist you and your family in meeting your health care needs. This provides our clients with access to the right care, at the right time, in the right place.
As part of Ontario’s Locally-Driven Population Health Model, CHIGAMIK is helping individuals and families in North Simcoe access the care, services, and community supports they need.
Through community outreach, service navigation, and one-on-one support from Community Ambassadors, this work helps reduce barriers and connect people to local resources that support health and wellbeing.
