Client Rights & Responsibilities
We support and care for our clients, their families, and the community with compassion, honesty, dedication, respect, and responsibility.
We will:
- Treat you with respect and courtesy
 - Keep your personal health records confidential
 - Give you time to talk
 - Listen to your concerns
 - Support you as you make important decisions about your well-being
 - Talk to you about getting a second opinion, if required
 - Explain what will happen if you consent to treatment or refuse treatment
 - Provide you with the best primary health care possible
 
You, as our client, have some responsibilities as well.
You are expected to:
- Be on time for appointments
 - Treat our staff and others at the CHC with respect and courtesy
 - Tell us about your health and well-being
 - Let us know when your personal information has changed
 - Ask us questions
